The Clinical Pharmacist prepares and monitors the use of various forms of medications, compounds, pharmaceuticals, medical supplies and equipment in the Clinical Pharmacy Division of the hospital and implements policies and procedures to ensure compliance with clinical practice and all applicable federal, state and local laws and regulations including New York State Department of Health. This position monitors therapeutic drug administration and safety, manages clinical pathways, formulary management, pharmacoeconomic, and reports patient outcomes utilizing detailed reporting systems. The Clinical Pharmacist attends medical rounds and makes recommendations for pharmaceutical treatments, manages drug therapy programs to ensure safe, appropriate and cost-effective use of medications in accordance with established clinical and safety procedures and regulatory requirements, monitors patients for adverse reactions and develops alternative pharmaceutical treatments. The Clinical Pharmacist provides information to physicians and qualified prescribers on the availability of existing medications, new drugs, warnings, and incompatibility of certain drugs and contraindications of medications and other pharmaceuticals.
Responsibilities:
Performs initial assessments of drugs having pre-established criteria to be monitored and monitors these drugs for follow-ups.
• Screens computer printouts for anti-infective regimens that the culture and sensitivity test show to be inappropriate, or for other drugs that show inappropriate use, or for doses that are inappropriate due to organ dysfunction (e.g., renal and/or hepatic dysfunction).
• Retrieves, analyzes, evaluates, and interprets the scientific literature, and serves as the resource person for patient and population-specific drug information to health providers.
• Prepares medications to prescriptions by qualified prescribers in accordance with established hospital protocols, New York State Department of Health regulations for dispensing and storing medications and controlled substances and other applicable regulatory requirements including 340B and USP797 compliance and taking into consideration contraindications, allergies, dose, route, frequency, duplicate therapy and formulary status.
• Interprets, clarifies and rectifies physician medication and IV orders, including the medication reconciliation process for inpatients and outpatients.
• Contacts physicians to verify patient medication orders for dose, route, name and frequency of administration.
• Prepares IV admixtures including chemotherapy in accordance with established aseptic methods.
• Compounds sterile and non-sterile preparations in accordance with compounding regulations.
• Monitors all patients on specified target drugs and initiates pharmacokinetic dosing protocols.
• Evaluates drug therapy for appropriateness and makes drug therapy recommendations to clinicians.
• Prescribes drug therapy per collaborative drug therapy management agreements and Medical Executive Committee approved protocols.
• Dispenses the final product for administration, instructs patients on the proper use of medications and performs patient counseling.
• Identifies, monitors, and reports adverse drug events and medication errors in accordance with established procedures and regulatory requirements.
• Participates in development of drug protocols and clinical pathways.
• Monitors all patients on specified target drugs and initiates pharmacokinetic dosing protocols.
• Evaluates drug therapy for appropriateness and makes drug therapy recommendations to clinician.
• Identifies, monitors, and reports adverse drug events and medication errors.
• Attends and participates in various clinical meetings for the hospital.
• Acts as liaison with the Director of Pharmacy and Medical Staff. Participates in development of drug protocols and clinical pathways.
• Performs drug usage evaluations on selected drugs.
• Utilizes specialized software on automated systems to perform drug usage evaluations on selected drugs and prepares and submits required reports.
• Completes quality assurance auditing as required.
• Dispenses controlled substances and maintains proper distribution and inventory records in accordance with established procedures and regulatory requirements.
• Ensures de-escalation for IV antibiotic therapy to oral antibiotic therapy.
• Implements the medical reconciliation process.
• Purchases medications utilizing in accordance with contractual agreements.
• Performs other duties as assigned.
Qualifications/Requirements:
Experience: 3 years’ experience as a Pharmacist in a Hospital setting required or completion of PGY 1 residence. Completion of PGY2 residence preferred. Experience with CPOE system preferred.
Education: Doctorate in Pharmacy (PharmD) required.
