Dublin Post Acute Care is seeking a Full-Time MDS Licensed Practical Nurse (LPN) to join our dedicated interdisciplinary team. Our facility is committed to providing high-quality, resident-centered care in a supportive and collaborative environment.

Position Summary:
The MDS LPN plays a vital role in coordinating and completing the Minimum Data Set (MDS) assessments to ensure accurate resident documentation and compliance with state and federal guidelines. This position works closely with nursing, therapy, and administrative teams to promote the highest standards of care and resident outcomes.

Key Responsibilities:
  • Complete and submit MDS assessments in compliance with all state, federal, and facility regulations
  • Collaborate with the interdisciplinary team to collect and review resident data
  • Ensure accurate and timely documentation in residents' medical records
  • Monitor care plans to reflect residents' current needs and goals
  • Participate in quality assurance initiatives and care conferences
Qualifications:
  • Current, valid LPN license in the State of Ohio
  • Previous experience with MDS 3.0 assessments preferred
  • Strong attention to detail, organizational, and communication skills
  • Ability to work independently and as part of a collaborative care team
  • Skilled Nursing Facility experience is a plus
Schedule:
  • Full-Time, Monday-Friday, Day Shift
