Job Description

We’re Hiring for a Part-Time Nurse Join our compassionate team at Griswold Homecare, a reputable home care agency, as a Part-Time Registered Nurse responsible for conducting in-home assessments and developing personalized care plans. We will need 3-4 assessments done per month. Must be flexible during weekdays. Responsibilities

• Conduct initial and repeat RN Assessments for our clients
• Develop Plans of Care based on the client’s needs and services requested
• Supervise provision of care according to the Plan of Care and the performance of the home health aide(s)
• Review and make reasonable inquiries to determine changes in client status periodically
• Communicate with authorized family / personal representatives in the care of the client when needed
• Document all changes in the client’s medical condition
